Dr. Nath is a Board-Certified pediatrician who believes that effective communication between patient and physician and preventative care are crucial to ensure quality patient care. She has special interests in pediatric medicine which include newborn medicine, vaccine counseling and adolescent screenings. Dr. Nath received an award in research in her studies of HPV vaccination at UCSF Fresno’s outpatient clinic. She advocates for preventative health and outreached to students regarding nutrition and STD prevention during her time at UCSD. Before joining our health center, Dr. Nath worked at University of California Fresno and Community Regional Medical Center practicing inpatient and outpatient medicine with an emphasis on newborn medicine. Outside of medicine, she enjoys yoga, painting, travelling and spending time with her puppy.

Dr. Moucharrafie is a Board-Certified pediatrician whose experiences demonstrate a comprehensive understanding of health. She recognizes that nurturing community health involves not only medicine, but also education and mentorship. She has found numerous opportunities to mentor medical students and work with community programs to provide health education to youth on topics ranging from nutrition to emotional well-being. Before joining our health center, Dr. Moucharrafie worked as a pediatrician at various community clinics and hospitals in Riverside County caring for acutely and critically ill pediatric patients. Aside from practicing medicine, she enjoys playing instruments, solving puzzles, swimming and photography.

Dr. Nielsen is a Board-Certified Pediatric Physician, who offers a compassionate, modern and detail-oriented view into patient care and is passionate about the safety, health, and well-being of kids: “Children are the future!” She has extensive education from the University of California, Irvine in public health sciences and public health policy, where she competed collegiately in Cross Country and Track & Field. She then earned her medical degree at the University of Arizona, Tucson and received distinction in Integrative Medicine. She completed her Pediatric Residency at the Harbor UCLA Medical Center where she received awards in Advocacy and Humanism. After residency, she gained additional experience as an Attending Physician for Children’s Hospital of Los Angeles and Children’s Hospital of Orange County in both inpatient and outpatient settings. Outside of medicine, she enjoys running, hiking, curling, swimming and spending time with her family, friends, and pets.

Dr. Andrade was born in the Philippines but grew up in Whittier, California. She is the first in her family to go to college and to become a doctor. She studied Molecular and Cell Biology and Psychology at UC Berkeley. She is passionate about and committed to serving underserved communities because she grew up without access to medical or dental care and saw the effects of it on the health of her family. She enjoys being able to take care of any individual who needs it whether they are children, men, women, elderly, or pregnant which is why she chose to attend the number one ranked medical school in primary care, the University of Washington School of Medicine in Seattle, Washington. She then moved back to Southern California for her Family Medicine residency at Long Beach Memorial Medical Center where she did extra training in obstetrics and maternal care and was the Chief Resident in her final year.
She believes in the power of education and likes to make sure patients understand the care she is providing so that her patients feel empowered to play an active role in their own health care. She provides preventative services to adults and the elderly, HIV preventative care (PrEP, PEP), prenatal care for uncomplicated pregnancies, women’s health services like IUD and Nexplanon placement as well as pap smears for cervical cancer screening.
In her past life, she also tutored organic chemistry and served as an educational counselor to first generation college students interested in the STEM fields. She speaks some medical Tagalog and Spanish. She is married and has two young boys. When not in the office, she spends her time with her family and dreams about traveling. She enjoys concerts, comedy shows, plays, running, baking, and then running some more to eat what she bakes.

Dr. Kavosh Rahimi, M.D., is a considerable and dedicated Internal Medicine physician certified by the American Board of Internal Medicine with extensive experience in Primary Care, Internal Medicine over the past 25 years.
He is a graduate of Yale School of Medicine affiliated Internal Medicine Residency Training Program in Danbury Hospital. He has taught residents in Primary care track program in Danbury Hospital. He was a clinical assistant professor in the department of Medicine at the university of Vermont college of medicine.
Dr. Kavosh Rahimi is a considerate and dedicated internist serving the community of La Habra. Dr. Rahimi attended the Tehran University of Medical Sciences School of Medicine in Iran, where he received his medical degree. He then completed both his internship and residency in internal medicine at Danbury Hospital.
Dr. Rahimi is a member of the American College of Physicians, the American Medical Association, and the Iranian American Medical Association. Dr. Rahimi welcomes patients who speak English, Farsi, Kurdish, and Turkish. His awareness of these different languages makes him accessible to patients from a variety of cultural backgrounds. Dr. Rahimi is sensitive to the uniqueness of each.

Dr. Wang is an experienced, skilled, and trusted pediatric dentist, focused on providing excellent dental care for her patients. She understands not every patient has a positive dental experience, which is why Dr. Wang’s goal is to be as gentle as possible. She is proud to offer comprehensive and individualized oral health care to children, starting from infancy and leading up to adolescence. Practicing dentistry since 2002, she is currently the Dental Director at Friends of Family Health Center, and has been since 2009.
Dr. Wang’s Master’s Thesis allowed her the opportunity to develop a visual aid, used to educate parents of young children, in order to explain children’s dental treatment needs. Her discoveries displayed the use of this visual aid improved the understanding of dental care. They also significantly amplified treatment cooperation of the parents, as well as their children. Dr. Wang’s Master’s Thesis is published in the Pediatric Dentistry journal.
Dr. Wang lives in Fullerton with her husband, and they enjoy spending their time traveling together, discovering new places. They also particularly enjoy dining out, cooking, and spending time with their loved ones.

Dr. Charlanne Holman attended Loma Linda University and received a Bachelor of Science degree in psycho-biology in 1988. She continued her education at Loma Linda University School of Dentistry and received her Doctor of Dental Surgery degree in 1992.
Dr. Holman is married and blessed with a son and daughter. The family enjoys swimming, off-roading in their Tahoe, taking walks with their dog, and spending time with extended family. She grew up in La Habra and spent her summers dental assisting in her father's Whittier dental practice. Being part of the La Habra community is a joy and feels like home.
During her time as a dental student at Loma Linda University, Dr. Holman was involved with missionary service to Mexico and received a certificate of recognition from the American Dental Association. She has volunteered dental services for the Assistance League and the Peace Corp. Dr. Holman is a member of the American Dental Association, California Dental Association, and the San Gabriel Dental Society.

Dr. Farzad Farhat attended the University of Maryland and majored Bachelor of Science degree in Chemistry. He continued his education at the University of Maryland School of Dentistry and received his Doctor of Dental Surgery degree in 1985.
Dr. Farhat is married and blessed with children of his own. As a dedicated husband and father Dr. Farhat values the importance of family. In particular, as a parent he strives to be an example of service, compassion, and duty for his children and the community.
During his time as a dental student at the University of Maryland, Dr. Farhat dedicated himself to understanding the theory and practical application of comprehensive oral health. Upon completion of his training he applied his expertise to serving the community of Beltsville, Maryland. He then traveled the nation to start his independent practice in Ft. Lauderdale, Florida where he forged strong ties with the community. Later on in 1989 Dr. Farhat sought new opportunities to serve and expand his practical knowledge by moving to the golden state of California. Since his move he has established a well-known reputation for passionate oral health care.
Throughout his extensive work history Dr. Farhat has been recognized and accepted as a full-fledged member of both nationally and locally recognized dental associations including the American Dental Association, California Dental Association, and Orange County Dental Association.

Dr. Rahimi is married and has 3 children who are all in the medical profession. His son is a liver transplant specialist out of Dallas, one of his daughters is a family practitioner and his youngest daughter is a pathologist at Northwestern Chicago. Her passion for mental health has made her a key provider at Friends of Family Health Center, providing psychiatric assessments and medication management to a wide array of the patient population. She received her Master of Science degree in Psychology from Central Missouri University and her Physician Assistant degree from Wichita State University. In 2015, she received an Employee of the Year Excellence Award for outstanding service to her clients. Originally from Kansas, Diane came to Sacramento in 2017 where she continued working in community mental health. Diane joined Friends of Family Behavioral Health thereafter, continuing to provide excellent and thorough care to her patients. Outside of practicing medicine, she enjoys photography and spending quality times with her dogs.

Dr. Ongcapin is a licensed psychologist with over 18 years of experience in community mental health. She has provided mental health services to adults, children, and adolescents. She also has experience working as a medical social worker for the County of Los Angeles. She has a private practice in the City of Covina and her specialty is in working with individuals with anxiety and depressive mood disorders.
