Notice of Data Security Incident

 

Friends of Family Health Center is committed to protecting the privacy and security of your personal health information. We wish to inform you of a data security incident involving one of our service partners. We understand that news like this can be concerning, and we want to share what happened and what steps are being taken to support you.

What Happened?

Friends of Family uses an Electronic Health Record system provided by OCHIN. OCHIN works with TriZetto Provider Solutions to support health insurance processing services. In this arrangement, TriZetto provides services to OCHIN and, indirectly, to Friends of Family. On October 2, 2025, TriZetto identified unusual activity within one of its systems and promptly secured the environment. Cybersecurity experts were engaged to investigate, and law enforcement was notified. The activity was contained, and there has been no evidence of further unauthorized access. The investigation determined that, beginning in November 2024, certain historical insurance-related reports stored in TriZetto’s system contained information about some Friends of Family patients and were accessed without authorization. Only a limited number of Friends of Family patients are affected, approximately 2,200.

What Information was Involved?

The information involved varied by individual and may have included:

  • Name
  • Address
  • Date of birth
  • Social Security number
  • Health insurance information
  • Other basic health or insurance-related details

No bank account, credit card, or payment information was involved.

What are we doing?

Friends of Family has worked closely with OCHIN and TriZetto to understand the scope of this incident. Some of the information received was incomplete, including missing addresses and internal record numbers. Notifications are being sent as contact information becomes available. TriZetto has taken steps to strengthen system security. In addition, TriZetto is making complimentary 90-day credit monitoring and identity protection services available to affected individuals through Kroll Credit Monitoring Services. You will receive a letter soon, containing an enrollment code and member ID. You can then go to enroll.krollmonitoring.com to get started. What You Can Do

As a precaution, we recommend that you:

  • Review your medical statements and insurance notices for anything unfamiliar
  • Monitor your credit reports at all three major agencies. There is no cost to sign up and request a copy of your credit report. You may also "freeze" or lock your credit against unauthorized access.
