On the login screen, there is a "Forgot Your Password?" link that will ask you for some personal information. You will then be able to set a new password for the Patient Portal. Alternatively, you can contact FollowMyHealth support at 888-670-9775 or support@followmyhealth.com for assistance.

Your personal health information is very secure. The FollowMyHealth Patient Portal is HIPAA-compliant in that it adheres to mandated encryption standards when receiving, sending, and storing a patient's health information. Information shown within the Patient Portal is merely a snapshot of your most recent medical records, not the actual medical record as a whole.
It is likely that your email address on file with the Patient Portal is not documented correctly. Please call us, Friends of Family Health Center at 562-690-0400, to verify the correct email address. Unfortunately, we do not have any way of knowing if the email reminders and updates from the Patient Portal are successfully reaching your email inbox. Also, be sure to check your email account's Spam folder. Some email clients use strict filters which can mistakenly classify communications from the Portal as spam and place them in this folder. If this is the case, be sure to denote this communication as "Not Spam".
Proxy or Dependent Accounts allows you to securely communicate with a patient’s physician office on behalf of a family member or person under your care. This communication is done using a FOFHC Patient Portal account. For pediatric patients, we will extend Proxy invitations to our FOFHC Patient Portal to parents/legal guardians named on FOFHC’s Pediatric Registration Form. FOFHC will scan your ID and inform you of any other documentation necessary to provide access. We apologize in advance for this inconvenience, but it is necessary for security purposes.

These types of accounts allow the parent caregiver or guardian to securely access information on behalf of a pediatric patient under the age of 12 years old. On the patients 12th birthday it will automatically terminate due to privacy concerns.
These types of accounts allows access to help manage adult patient care who is 18 years of age or older. In order to obtain access, the patient can invite another person, or provide FOFHC a notarized letter allowing the Adult Proxy permission to obtain access. These steps are required to ensure that we allow access to people whom you want to have access to your personal medical records online; it is necessary for security purposes.
A patient can also grant proxy access to their own FOFHC Patient Portal account to another person.
